Registration Forms

Registration Forms are due in the Church Office by June 6th.  No Registrations will be accepted after this date.

Confirmation Retreat

What does it take to build a church?  For these students, creativity along with pasta and marshmallows!!

This group had the highest, strongest, most beautiful freestanding structure.

The group listens to instructions before hitting the water at the USNWC.  What a beautiful, fun and faith-filled day!

2008 - 2009 Faith Formation and Sacramental Program Registration Information and Forms

Please note: Families must be registered members of St. Patrick Cathedral to participate in all Faith Formation and Sacramental Programs. If you are not yet a member, please email Mrs. Louise Bowers for additional information at LBowers@StPatricks.org.

Baptismal Forms
- If your child was baptized at St. Patrick's, please provide the month, day and year of baptism.
- For children baptized outside of St. Patrick Cathedral, a copy of the baptismal certificate is required at the time of registration.

Category 1 - Faith Formation Program for Children Attending Public or Private School. This is for children who are not attending Catholic School who will be attending Faith Formation at this parish. Please print and complete the forms attached.

Category 2 - Children Attending Catholic School 2nd Grade during 2008-2009 School Year. Your child is will be prepared at school to receive the sacraments of Reconciliation and Eucharist and is not required to participate in our Faith Formation Program. However, the Diocese of Charlotte requires that children be registered and parents participate in formational opportunities made available by the parish. Please print and complete the form attached.

Category 3 - Teen Attending Catholic School 8th Grade during 2008-2009 School Year. Your teen is required to participate in Confirmation preparation per diocesan policy. Our Confirmation program meets each Sunday February - May from 9:15 to 10:30 a.m. Parents are also required to participate in formational opportunities made available by the parish. Please print and complete the forms attached.
